House Visit Registration Form

If your dog is coming to stay, be it overnight or just for day care, please fill out the “Boarding Registration form”.

If we are walking your dog for you from your own house, please fill out the “House Vist Form”.

Please fill out all the required fields on the form. It enables us to look after your pet safely, with all the knowledge we need. We are fully licenced with St Helens Council and our forms comply with their licencing requirements protecting you too. Please read the terms and conditions and sign at the bottom.

Filling out the form covers all future visits. We will keep the details on record for that purpose so you only have to fill it for the first visit.
